It probably varies by state, but I live in North Carolina and had the same problem. There were two choices: (1) ask them to mail (snail-mail) a form, which took weeks, and (2) look it up on the Department of Employment Security site for the state of NC, using the account I used to file for unemployment benefits. I found this easy and quick, less than 5 minutes. I would suggest you try google for your state. You may be able to do the same.
Many states do not mail Form 1099-G to unemployment recipients - they are made available on the state's unemployment commission or Department of Labor's website.
