How do I fill out Part I, box 7 of 2848 (Rev 1-2021) when signing as a POA

In regards to the signature for 2848 (Rev 1-2021) form when signing as a POA:

When filling out a 2848 (Rev 1-2021) form, and attaching a POA to it for a parent who is unable to sign their tax return due to injury, how should I fill out the following lines in Part I, box 7 of form 2848?

How do I fill out these lines?

Signature: (would this be my name?)

Date: (Date that the above signature is signed)

Title: (Would this read “POA for ‘parent’s name’”?)

Print Name:  (would this be my name?)

Print Name of Taxpayer from line 1 if other than individual:  (my parent’s name)

If you are completing a power of attorney for someone else who is authorizing you to sign for them, then it would be their signature on the signature line.

The box 7 questions you are asking above is asking for information of the taxpayer appointing you as their power of attorney person on all lines.

"Title" may not be used unless their name does include a "title".

How do I fill out Part I, box 7 of 2848 (Rev 1-2021) when signing as a POA

I am assigned as the "attorney-in-fact" in a general durable POA for my parent (my parent is not able to sign the return due to illness/injury).  So, I was thinking that I was suppose to sign with the title "POA for 'my parent'", but maybe that is not correct.

So, would this statement be true?

Line 7 – Signature of taxpayer:  Your parent can sign the form if he or she still is competent and capable of signing. If not, you can sign as power of attorney then list POA for [parent’s name] as your title, then print your name and the name of your parent for whom you are signing.
