In your Massachusetts return, you will reach the page that says: Let's Go Over Your Health Insurance Information. This will bring you to the area where you will enter the information from your 1099-HC.

Note: If you have private insurance, a Masshealth Plan, a Commonwealth Care or a ConnectorCare Plan, most of what is needed for this section of your state return comes from Form 1099-HC, sent by your insurance provider. (If you have government subsidized insurance, you won't always get a 1099-HC.)
