You can use this link which has instructions on entering your 1099-G.   Where do I report a 1099-G for unemployment or paid family leave?

How do I file a 1099G rev.36 disability

The 1099G rev36 is a different format than the 1099G (i.e., there are different boxes and two tables). The link provided for the 1099G previously in this message feed does not address how to file the tax rev36 document. Please provide instructions on how to do this in Turbotax. Thank you!

How do I file a 1099G rev.36 disability

If you show Table A and Table B, and IF they both have different information on them, enter them both as if they are two separate 1099-G Forms. If only one is reported, just enter the information from that one. 

Next, make sure you are in the correct section in TurboTax. There is a 1099-G section for reporting taxable Grants and also for state refunds, you don't want to be there. 

Please type   1099-g unemployment   into the search box at the upper right corner and then click the "Go to 1099-g unemployment" link. 

(DON'T GO TO 1099-G STATE AND LOCAL REFUNDS)

Next, enter the information from your 1099-G rev. 36 into the TurboTax screen.
