How do I complete missing information for form 8995 and refile?

Turbo tax isn't giving me the option to find/fill out the form 8995

First a prior year return must be paper filed at this late date.  And the program fills in the form 8995 automatically if you are qualified for the QBI deduction.  Sometimes the form is not needed and only a worksheet is produced. Review the return in the FORMS mode to see what is happening with that situation.
