The 1099-DIV and 1099-B do not have that info listed. Normally, it can be found on annual reports such as those from mutual funds and ETFs.  For federal income tax purposes, I it doesn't matter which state they came from.  They are all federally tax-exempt.  I downloaded my 1099's from Charles S***** and it populated "Various" in the state box. I have also done that manually in years past and never encountered an issue.

However, it may matter on your state income tax. There are some states that issue muni bonds that are also tax-exempt on that same states income tax e.g. New York state muni bonds are tax-exempt for those filing in NY.  If you own a mutual fund of muni bonds and live in NY, you need to get the % of those bonds issued from NY.  As I said, I have found that info on annual reports.
