When did you enter USA with J-1? Did you have any days present during the prior two years? What visa are you on now? Did you receive a W-2 for the work or what ?

My Colorado state tax return has not come via mail, there has been a complication with the mailing address. How can I amend the mistake and receive my cheque? Thank you.
