You will need Form 1095-A to
create the Form 8962. The Marketplace uses Form 1095-A to report certain
information to the IRS about individuals who enrolled in a qualified health
plan.
It’s important to remember the
following:
- Read the details of your IRS letter carefully and follow the
instructions provided in it.
- Only print and send the forms requested by the IRS in your letter.
- You won’t be completing or sending a Form 1040X (amended tax
return) to the IRS.
TurboTax Online
Instructions
Follow these steps:

Sign in to TurboTax with the same login you used when
you filed your 2016 return.
- On
the Welcome home screen, select Taxes.
- On the Tax Timeline, select Amend
return.
- Select Amend using
TurboTax Online.
- On the We'll help
you change (amend) your return screen, select the Federal
Taxes tab.
(Don’t select Continue. You don’t
want to start the amend process.)
- Select the Health
Insurance tab.
- Answer the questions about
your health insurance which includes entering your 1095-A.
- On the Do you need
to change anything else screen select Done.
- Select My Account at the top of the
screen and choose Print Center.
- Choose Print, save,
or preview this year’s return.
- Select View or
print.
- Select Form
8962 and
either Form 1040 or 1040A and print them.
- Send the following to the IRS
address or FAX number given in your IRS letter:
- Form
8962, Premium Tax Credit
- Copy of your Form 1095-A,
Health Insurance Marketplace Statement
- A copy of the IRS letter
that you received.
