Form 8615 Dependent Living With Custodial Guardians

My grandson lives with me by court order.  His dad has passed and left an insurance benefit for him.  It was put into an Fidelity acct and it earned dividends, a small amount of capital gains and that investment fund paid some foreign taxes.  He also received an interest payment on the insurance money while it was being held by the insurance company.  As a result he has about $3300.00 in unearned income.  So on Form 8615 do I put the guardian's name and taxable income down in the blanks that ask for the parent's info?  The birth mother is out of the picture

Form 8615 Dependent Living With Custodial Guardians

Yes, if you are his legal guardians, you would enter your information on form 8615.
