Spouse and I are splitting gifts on the 709. I am the one that gave the gifts out and each donee exceeds our combined exclusions amounts. IRS directions indicates this will require both of to file and each consent to the other. Does this mean I use a mirror image of my form for spouse, or I split the gifts between the separate forms?

Correct. You will both file a return and consent on each other's 709.

Understood, so are the returns essentially 1/2 of the gift(s) total on each or full gift amounts amounts on each and filling out (i) on the form?

You enter 1/2 of (g) on (h). Subtract (h) from (g) on (i).
