After completing all the information on Form 4852, the form failed TurboTax's review because the recipient and/or taxpayer's address was a foreign address.

A trustee-to-trustee transfer of an IRA is not a reportable transaction, so nothing about it is to be reported on your tax return or entered into TurboTax.  Delete the substitute Form 1099-R (Form 4852).

To clarify, what type of income is this?

We have transferred all funds from Company A's IRA account directly to another IRA account at Company B.

After the transfer, Company A closed the account, so we cannot obtain a copy of Form 1099-R. We need to complete Form 4852 as a substitute for Form 1099-R.

When previewing the form, line 3, "Address," correctly displays the current foreign address, which is the same as the address on Form 1040. However, the TurboTax software reports an error and prompts us to correct the address. The software only allows us to enter a US address for correction. Even when we try to enter a US address in the "Let’s Check These Entries" section, the software still reports an error. We suspect this is because the entered US address does not match the foreign address shown on Form 1040.

Note that From 4852 cannot be e-filed.  If your tax return includes Form 4852, you must print and mail your tax return.
