Form 14039 (Identity Theft Affidavit)

My federal and state returns have passed their reviews are ready to be submitted.  TurboTax is telling me I can't proceed because I need to submit a Form 14039 (Identity Theft Affidavit) and the form is not ready.  What do I need to do to proceed?  Will I have to submit my return manually?

14039 is not a form included with your return. as a matter of fact if you were a victim it's possible your return will be rejected if you try to e-file. read the instructions at the bottom of the form.
