How do I report Foreign Dividend Income if Foreign Financial Institution doesn't have a FEIN?

The 1099-DIV entry in TurboTax does not require you to enter an EIN.

also the address of the payer is not needed. you may need to delete this so Turbotax doesn't bug you about the missing EIN
