I have a QDRO 401k I took money out of in 2018. I did pay the federal tax on it at the time of the withdrawal. So Box 4 is the tax amount I already paid, and the Distribution Code is 2 since the QDRO is exempt from the early withdrawal penalty. Should Box 1 and Box 2a be the same amount? If I took out $10k plus $5k for federal tax should both boxes be $15k total? And should I be putting anything in the other boxes?

best advise is type the information from the form into Turbo Tax EXACTLY as its on the form ; if the other boxes are blank, leave them blank...don't over think it!
Assuming the cost basis is zero, you in effect received $15,000 and had $5,000 withheld for taxes. I'd expect both boxes 1a and 2 to be the same - $15,000
I have a similar situation. I have a QDRO 1099-r, but Turbo Tax indicates no tax is due. Recipient is less than 59 1/2 and receiving a share of EX's retirement fund. 1099-r was entered a received. I expected to pay tax. Can this be right?

i checked your response in TT Desktop Deluxe and it worked correctly.
Are you using the Online Version or the Desktop version?
Do this: remove the 1099-R form and review Form 1040. lines 11- 22. Write down those numbers. Then add the 1099-R form back in. Line 11 should definitely be higher. what happens to the other line items?
