The filing instruction letter does not go to the IRS. Why are you worried about it? It is only for your own records.

And...tax due was supposed to be paid by May 17, 2021 even if you have an extension to file, so the filing instructions keep that date. As VolvoGirl suggested, you can cross out the May date and put in the October date that you file since it is only for your own records to remind yourself that you did not file until October when you look back at it someday.

No. It's not important. It's only for your files. Ignore it.

No. Again----that instruction sheet is only for your records. Look at your tax return. What date do you see on it? No date, right? The IRS knows you have an extension if you requested one and they accepted the request. As long as you file by midnight on October 15, 2021 you are filing it on time.
If you end up printing and mailing the return then you have to write the date that you are actually filing it next to your name. If you e-file, there will not be a physical signature or date on the tax form on paper even if you print it out.

When you requested your extension (if in fact you did that) you were supposed to pay your estimated tax by May 17, 2021. The extension only gave you extra time to prepare and file the Form 1040. Even if you now owe more since preparing the return, if you had an extension you will not be charged interest or a penalty if you file by 10-15. If you did not have an extension and you owe tax due, your interest and penalties began when you missed May 17.
Trust the IRS's computers to know when they get your tax return and to use the right date.
Check 1040 line 38 for any penalty. And actually that is just an estimated amount. Turbo Tax won't know the exact date the IRS will get your payment. It's common for the IRS to send you a bill for more interest and penalty or a refund depending on when they get your payment.
