I'm getting two errors entering data for form 1099-R. First, it claims the entry for address type is invalid, even though it's chosen from the drop-down list. Second, it won't let me enter zero for line 9a, insisting the minimum value is 0.01.

Do not enter anything in box 9a, leave it blank. What address type did you select? Is this a US address or a Foreign address?
if the taxpayer's name and SSN on the 1099-R is correct why would none of the distribution be allocable to the taxpayer?
As I think Mike9241 is implying, a zero in box 9a is nonsensical since it would be saying that the Form 1099-R is being issued to someone who was not eligible to receive a distribution (and should not have received a distribution). Unfortunately, many payers incorrectly think that blank and zero mean the same thing on a Form 1099-R and mistakenly use them interchangeably.
