The steps below are from TurboTaxes instructions for entering Crypto Sales manually in the desktop version.
My TurboTax Premier is updated, but when I follow steps 1 - 4, step 5 "Select CSV, 1099-B, 1099-K, Gain/Loss Report on the OK, let's start with one investment type screen" is not the next screen. Instead I get "Did you get a 1099-B or a brokerage statement . . .", regardless of how I answer this, I never see the "Select CSV, 1099-B, ..." I'm told to expect in step 5. How do I get to that point?

Where did you find those instructions? I don't believe there is any reference to uploading CSV files in the desktop program - those instructions are only for TurboTax Online products.
But you can still do a manual entry in the desktop product. When asked if you received a 1099-B, if you answer NO, manual entry will be your only option. If you answer YES to the 1099-B questions, you will see a screen to import the document or type it in manually.
Thanks for the reply. The instructions were here: https://ttlc.intuit.com/turbotax-support/en-us/help-article/cryptocurrency/import-cryptocurrency-tra...
