E-file Form 1065 gets rejected for EIN/Name Control reasons. re- checked all info, retyped name & EIN fields as suggested, checked NAME or ADDR change box w/same results

My E-file From 1065 keeps getting rejected for EIN/Name Control reasons. Last year we used a CPA who eFiled our initial return. we have double checked all information and it matches last years, retyped name and EIN fields as suggested, and it is correct on the form. we have tried various combinations of checking the NAME change and/or ADDRESS change box with same results regardless. I'm aware we can print and mail the return, but we should be able to fix this issue and eFile.  Can you help?
