To clarify, what state is this and Box 14 from what? Is it from a 1099 DIV or Box 14 on a W2?

No, don't enter your Box 14 information with your NJ Dividend income. On the state dividend screen, enter the amount of your total dividend income that is attributable to New Jersey.
When you enter your W-2, box 14, use Other - not on list for the code for that line item (UNION & OCARE).
Ok thank you, I guess I just don't understand what is a dividend then.
If you own shares of stock in a company then that company will pay you a portion of their income each year. That is what dividends are. They would be reported to you on form 1099-DIV.
