Didn’t receive 1095 form

If you had marketplace health insurance you must get your 1095A and enter it on your tax return.   If you had some other kind of health insurance ---not from healthcare.gov--you do not enter a 1095B or a 1095C anywhere.

 

 

To enter your 1095A go to Federal>Deductions and Credits> Medical>Affordable Care Act (Form 1095A)

How to find your 1095-A online

  1. Log in to your account.
  2. Under "Your Existing Applications," select your 2023 application — not your 2024 application.
  3. Select “Tax Forms."
  4. Download all 1095-As.

Get screen-by-screen directions, with pictures (PDF, 504 KB).

Didn’t receive 1095 form

Some reasons why you may not receive an IRS Form 1095-A or Form FTB 3895:

  • You were enrolled in a minimum coverage plan (also known as catastrophic plan).
  • You were enrolled in employer health coverage plan

Did you or anyone in your household have Marketplace insurance.

Please confirm that you do not have any open accounts.

You can use the phone number by state at The Health Insurance Marketplace.
