CSV form

When I try an upload CSV form from coinbase it denies it due to "no header." Any suggestions?

That first link is invalid, there is no such option listed on Coinbase. What coinbase is doing is having a CSV file just to give people one so that their consumers can use cointracker which they get paid a commission for those that use that service. The CSV file isn't suppose to work in the first place. It wouldn't be any point to team up with cointracker if Coinbase CSV file worked.
