You would have to create a Form 4852 Substitute W-2 and you will not be able to e-file without an EIN. You will have to mail the return. The IRS procedure is below in the first link. after you enter the information that you have on the W-2 form, you will see the choices on the screen shot below. Click on Substitute W-2
