I thought when you start an amendment, everything starts back at zero. Why does the federal refund show $48 even before anything is entered?

Yes, generally the amended returns start at $0. However, if there was an update either in the Federal or State law, or in the program, that changed the amount of your refund, then it could start with a different amount.
Check your saved or printed copy of your return as filed and compare it with any refund or balance due statement that you received.
