After e-filing my state return with turbotax, I received a "filing instructions" sheet. It says I need to sign and date form IT-8453-OL. Do I mail it? How do I get it?

The 8453 that printed with the return has a "Do Not Mail This Form To DOR" watermark on the form. Where do I get a clear form? Who do I mail it to?

No, you do not need to mail this form. You should just sign it and keep it for your records. The form has do not mail printed in the top right corner. See my (screenshot) below for verification.
