401k rollover into IRA

Hi, I rolled 4 401k plans into a Traditional IRA.  How do I account for this and what forms do I need?

You should have received a Form 1099-R from the plan administrator who transferred the 401(k) funds to the IRA.  The Form 1099-R has to be entered on your tax return.  If you did not receive the 1099-R contact the plan administrator.
