The first page shows boxes 15 and 16 with your state information. Pages 2 and 3 just show the rest of your local earnings. If the local wages are correct, you can just keep adding lines to enter the local taxes on your w2.
If you add up all the local boxes, it should be equal to or less than your state wages. Sine you worked in multiple locations, there could be an error on your form. If the local earnings add up to more than your state tax, contact your HR department to get your w2 straightened out.
