1099R

I put in 1099 R an amount and 0. taxes in line 2a but it was added on my income. can someone help on this?

Did your paper form have an acutal zero on it or was Box 2a blank? A blank box is not the same as having a zero.What code was in Box 7?

What was the code in Box 7 on the 1099-R?

Is this from a retirement plan (and if so, what type)?
