I did use some money from my HSA but did not get a 1099-SA

Contact the financial institution that holds your HSA account.

You might log in to the website of your HSA plan administrator -- the bank or financial institution that issued you the debit card to pay for qualified medical expenses.  Generally, tax documents will be available online.

If not, try to reach out over the phone.  The administrator should be able to provide you with a copy of the 1099-SA.
