1099-R

If you received a Form 1099-R, you have to report it on your tax return.  Whether the amount of the distribution that is reported on the 1099-R is taxable depends upon what type of distribution it is, which is reported in box 7 of the form.  It is possible that the annuity generated income that was not sent to you and is still in an account under your name at the credit union.  If that is the case, you have to report the income and it may be subject to income tax.

For clarification, is the annuity qualified (held inside a retirement account like an IRA) or non-qualified?  What is the distribution code(s) in box 7 of Form 1099-R.
