what if in box 18 "Name of Locality" is not where you live or earned the income? For example live and earned Erie PA but it says New York City.  When I look at the complete list of localities on turbo tax there are only Michigan an New York localities.  The Payer is address is in OH and the Payee is in PA.

Under box 18, if your locality is not listed, report 'XXX'

This amount will be listed under line 5a of Schedule A Itemized Deductions but the state and local tax deduction may be limited.

Contact the issuer of the 1099-R to make sure that subsequent withdrawals are not treated the same way.  If the tax was remitted to New York City and you have no liability to New York City, you may request a refund.
