I was covered by an employer from Jan thru April and then purchased coverage thru the Marketplace.  It is asking for the SLCSP and Monthly Advance Premium Tax credit in the following two columns, where would I find that information?  I have that info for the marketplace, but only know the premium deducted from my salary for the time I was covered under my employer.

When entering the Form 1095-A you do not include any coverage that was not provided by the state Marketplace exchange or from healthcare.gov.  Leave the months where you had coverage from your employer blank (empty).
