A Form 1095-B is not entered on a tax return.

1095 B how do I file this

Do not enter a 1095B. You only enter a 1095A that is for having marketplace heath insurance from healthcare.gov. IF you have one. You do not enter a 1095B or a 1095C on your tax return.
