Answer Yes on the Did you receive Form 1095-A for your health insurance plan? ... Enter your 1095-A info on the next screen and select Continue. Hello,

I received five separate 1095-A Forms in 2021 because I got health insurance through the marketplace in 3 states. Please reference the list of states below to see the breakdown.

What section do I report this information on Turbo Tax?
Do I need to file separately for all five 1095-A Forms?

New Jersey - I had three separate 1095-A Forms because I changed my insurance 3 times while living there between 01/21-05/21

New Mexico - I received one 1095-A Form

South Carolina - I received one 1095-A Form

To clarify:

Would I do one for South Carolina and one for New Mexico?
Would I do three separate ones for New Jersey (since I have three separate 1095-A Forms from NJ)?

Yes. Do a separate one for South Carolina and New Mexico and then you will do three separate ones for New Jersey. As I said, do not combine them into one 1095-A entry screen.
