Form 1040/1040SR is your main Federal tax form.

If you received an overpayment amount back from your state, include this amount as part of your State Refund.

In the Wages & Income section, scroll down to 'Other Common Income' (screenshot).

Here's How to Enter a State Tax Refund. The interview doesn't ask for the Recipients SSN.

If you received a 1099-G in addition to amounts in letters, just add them together for one entry.
