On the Job Related Expenses section for Telephone line if I work from home, am I to document the monthly amount or total annual amount paid for telephone bill?

Job related expenses calculate by month or year

You will enter your total amount for the year. Just add the amounts together and enter that total. As far as documentation purposes, keep all the monthly statements.
