I paid $4,500 when I filed for extension and Turbo shows I paid $9,000 what am I doing wrong

Sounds like you entered it twice.   Did you enter it when you filed for the extension and then enter it again?  Check here and edit if necessary:

 Go to Federal>Deductions and Credits>Estimates and Other Taxes Paid>Other Income Taxes

Yes, if you used Turbo Tax to do the extension it automatically entered the amount on your tax return and then you added it again manually.  Have you filed yet?  If you already filed the IRS will catch that and adjust your return.
