Why is page 2 of form 2441, Child and Dependent Care Expenses, missing from my documents?

I've printed all documents, however page 2 of form 2441 is completely missing?

Page 2 is only needed if you had a flex spending account that paid child care expenses ... this would be on the W-2 form box 10.
