you don't get it from Turbotax. the form comes from the HSA custodian. if you want to see what went wrong with your HSA contributions look at form 8889. this is where the allowable and excess amounts are computed. you'll probably have to pay the Turbotx fees first.
do you have the contributions made through your employer? - your's and any employer matching - code W box 12 on your w-2. these contributions carry to form 8889 and are not to be reentered by you on the form.
TT uses the info YOU enter to determine excess contributions ... so did you make the correct entries ?
