Form 8692 part 4

I am the listed recipient on a 1095-A i received. My mother is the only covered person in the policy. I am not claiming her as a dependent on my tax returns.

Do i have to file a form 8692?

 

What percentage of shared allocation should we/she put on form 8692 part 4?

You do not report it since your mother was not your dependent. Give the form to your mother or who ever is claiming her as a dependent. They would file it with their income tax return.

Should she put 0% or 100% as Shared Allocation in part 4 under my social security #? (Turbotax is asking to fill this section out)

As the only covered person, she is 100%. If she shared the policy with someone, she could be 50%.
