Do I need to fill out some forms or submit documents indicating that my child is totally disabled with Autism, and unable to work at all for tax purposes when I claim her as becoming adult disabled depending on my taxes? Ex: letters of full guardianship given to me by the probate court, psychiatrist statements that she is unable to make decisions, the degree of cognitive impairment, unable to live an independent life and take care of herself and own affairs, etc.
Also, do I need to fill out the support sheet and what form is that? Thank you for your help!

No, there are no tax forms to submit or documents needing to be submitted to prove that your daughter is disabled. You only need to have the documents and doctor's statement available if ever questioned by the IRS, which is rare.
There is no type of form you need to complete showing that you are providing support to your daughter. Just like there is no form needed to prove support to any other dependent child or relative you would be claiming on your tax return. Again, proving support would only be necessary if ever questioned by the IRS and they would provide you with the information on how to show that support.

Thanks @DoninGA for the reply! I'll just add to what you said based on my research on the IRS website.
Per the IRS proof is needed:
"You must prove that your child has a permanent or total disability.
To prove your child's disability, get a letter from their doctor, healthcare provider or any social service program or agency that can verify their disability." Click here to read more of this article.
Although you don't have to submit the proof with your tax return it seems that the IRS wants you to have this proof handy in case they ever need it or need to ask any follow up questions.
