Mileage

I'm a caregiver. Clients get 110 miles a month. We get paid for 110miles. Over the 110 we pay  for the extra miles out of our pockets. Can this be deducted?  Also can we itemize oil change, tires etc.?

Are you a W-2 employee or an independent contractor?   If you get a W-2, then, sorry, no you cannot deduct job-related expenses on a federal tax return.   If you are self-employed you can enter business expenses on a Schedule C, which would include your mileage.

W-2 employees cannot deduct job-related expenses on a federal return.  Job-related expenses were eliminated as a federal deduction for W-2 employees by the tax laws that changed for 2018 and beyond.  Your state tax laws might be different in AL, AR, CA, HI, MN, NY or PA.

If you live in a state that lets you deduct job-related expenses, the information will flow from your federal return  to the state return, so enter it in Federal>Deductions and Credits>Employment Expenses>Job-Related Expenses

Q. Also can we itemize oil change, tires etc.?

A.  No. The mileage rate already  includes all automobile expenses.

No. You either get/take the per-mile deduction or actual expenses. Since you are paid per-mile and claim per-mile, you can't make any other claims for vehicle expenses. Do note also that if you're a W-2 employee then none of your job related expenses are deductible including the per-mile amount.
