I purchased items for Operation Christmas through church. Is this a donation and who do I list as the charity I donated to?

When you say you purchased items through church, did you receive goods or simply give money?  If you received goods, then you did not make a charitable donation unless you paid extra money over and above the fair market value of the goods.  

If you did make an additional donation, then you would list the church as the charity you donated to.
