You will be asked for the childcare provider's SSN or Tax ID and for the amount you paid the childcare provider to take care of your child so you could work. Save your records.
Yes, it should be proof enough. The bank withdrawals should be consistent with the dates of payment to your caregiver. Review the information below to be sure your caregiver is eligible as well. Also, rules for whether it was done in your home, their home or a daycare facility.
