i have over 16000.00 in medical bills in 2024. why is it only counting a portion of that ?

the $16K is reduced by 7.5% of line 11 (adjusted gross income) of your return.

Medical expenses are not fully deductible.  They are only deductible as an itemized deduction to the extent the exceed 7.5% of your AGI.
