I am a professional scuba diver & writing off some gear and mileage. Got no W-2 from the shop as they paid me under the amount they have to claim. Is this an issue?

Assuming you are using Home and Business: Just enter the amount they paid you under Business, Miscellaneous Income, and describe it as Used Equipment Sold. You can enter the shop info if you want but it's not necessary. Mileage is an ordinary Business expense. Turbo Tax will calculate this for you.
