How do I amend/correct 2441?

I entered the wrong information in Box 12 of Form 2441 under dependent care benefits. I didn't receive any dependent care benefits. I set some aside through my employer but didn't file the form soon enough to receive the benefits so they were forfeited. How do I amend 2441 or should I file an amedned 1040x?

You will have to amend the tax return to change the tax return that was filed with the incorrect Form 2441.

Box 12 should be the amount in box 10 of the W-2 and then box 14 should show the amount forfeited ... so is it box 14 that needs fixing ? https://www.irs.gov/pub/irs-pdf/f2441.pdf

Yes, it's box 14. I apologize for not being more clear. I saw the dependent care benefits in box 12 as what I didn't get correct and wrote the post. I didn't realize box 14 was related to forfeiture.

As mentioned above you will amend the return ... in doing so you will revisit the Day Care credit section to make that entry ... go slow and read the screens.
