Form 8995

I had a 1099 DIV with $2.34 indicated in Box 5 (199A) Turbo printed out a form 8995 but came up with 0 deduction and put 0 on 1040 line 10. Do I need to include the form 8995 with my tax return?

SECTION 199A DIVIDENDS. I have $3.52 in 1099-DIV, Line A. Do I check the box for them? I think this is why it is completing form 8995, making it look like I'm a business.
