Sales Tax per state of residence:  What do I need to put for Date Lived in State From and Date Lived in State To?

If you lived in the same state for the entire tax year, you will enter 1/1/2017 - 12/31/17.  If you lived in different states, then you will enter the dates you lived in each state.
