My adult son has a Medicaid waiver and I am a paid caregiver. federal taxes are not taken out of my pay. Do I just file a state income tax return?
I am treated as an employee with the agency that handles his waiver, but they stopped taking fed income tax . This is the first year I didn’t pay FICA so I am confused on what to do.

These payments are excluded from income. However, if you have received a W-2 that shows that either Federal and/or state tax were withheld from your payments, then you will file a tax return to have those withholdings returned. Here's an FAQ with additional information on this: https://ttlc.intuit.com/replies/4360071
