This question has been removed as the accepted answer did not, in fact, answer the question.  Partly my fault as I did not specifically mention that I was looking for anyone that had been in similar situation.  (It was pretty obvious that I could reach out to the original requestor.)

You may want to direct your questions to the Massachusetts DOR itself.  It's unlikely that anyone here can tell you definitively what documents MA DOR will accept, or how they should be formatted.
